Your instructor has created a survey that asks you to rate the degree to which the various elements of the course have helped you learn the material. This is your opportunity to provide your instructor with feedback about how well parts of the course are helping you learn and how well you feel you understand the material. In order to complete the survey, you will need the course ID number and a course password. You may also need to submit an unique identification string or number (which are requested once you submit the course ID and password).

A word about your privacy:
Only your instructor has access to your and your classmates' responses to this survey. If your instructor has assigned an ID to you, your instructor can determine whether you have completed the survey (by checking the ID), but this system prevents your instructor from connecting your ID with your responses.
