Business Office Forms

Microsoft Word Format

You can fill in the form in Word, then print the form, or you can print the blank form, then fill it in by hand. Be careful if you decide to fill out the form in Word; if you type in the wrong spot, you can corrupt the form. When you are ready, obtain the necessary signatures, and submit the form to the Business Office. Note that you can save filled-in copies of Word forms on your local computer.

ATTENTION
Adobe Acrobat Reader cannot save PDF forms - you can only fill them out and print them.

Administrative Forms About the Form
Arrange meetings & conferences (in and out of state) in which meal expenses, AV equipment, meeting space cost, etc. are requested. From this form requisitions for meals or conference expenses are generated.
Consultant Travel Request Initiate payment for a consultant's travel.
Determination of Payment Type Allow the Business Office to select the payment type that is in compliance with University and federal requirements.
Limited grants: 144-KN67, 144-LL26, 144-LL18 & 144-MS63. Initiate approval and payment for non-UW participant support to individuals who do not render a UW service.
Initiate appointment process; provide appointment details.
Once a payment has been identified as a subcontract or subaward, this form is to be completed and sent to the business office for payment processing. .
Purchasing Forms About the Form
Equipment Request Form Purchase computer hardware, software, and peripheral equipment; also desks, chairs, file cabinets, tables, storage cabinets, etc.; also equipment such as cameras, projectors, etc.
Printed Materials Request Form Purchase books, magazine/journal subscriptions, videos, etc..
Services Request Form Order services, such as transcriptions, etc.
Supplies Request Form Purchase office supplies.

Travel/Workshop Reimbursement Form UW & non-UW employees complete this worksheet to assist in preparation of a TER or PIR.
Travel Pre-Authorization Form Initiate approval and travel process for UW employees.

How to Use These Forms

You can fill out these forms on your computer, then print them. Note that you must have Adobe Acrobat Reader on your computer. To use these forms, follow these directions:

1. Click the link for the form you want. Acrobat opens the form.

2. Move the mouse around the form. Notice that the hand cursor changes to an i-beam cursor on top of the fields that you can type in. The fields shaded in gray are for the Business Office; you can't type in those. You also can't type in any field labeled Signature.

3. Click on the first field that you want to complete. Type your information, then press Tab to move to the next field. 

4. When you have completed the form, click the button labeled Print Form. This button is located on the form itself.

5. Obtain a signature in the signature field (if the form requires one), then deliver the form to the Business Office.

Hints

  • You still need a regular signature. Some forms have a field labeled Signature. You can’t type in this field. It must be signed by the appropriate person (indicated on each form). Not all forms require a signature; if this is the case, just fill out the form completely, and return it to the Business Office.
  • Have all your information ready, because you can't save a copy of the form. Acrobat Reader can’t save forms on your computer. This means that you can’t start to fill out the form, save it, and return to complete it later. You must fill out the entire form in one session. However, if you get started on a form, then need to stop before it's finished, you can print the form at that point, and finish filling it out by hand later.
  • Change your mind? If you start to complete a form, then don't want to do it after all, just close Acrobat by clicking the Back button to return to your browser.
  • Hate to fill out forms on line? If you really don’t like the idea of filling out the form on your computer, you can click the Print Form button without entering any data, then complete the entire form by hand.
